NRC INTEGRATED INSPECTION 50-333/98 07 AND NOTICE OF VIOLATION

Dear Mr. Colomb:

- This letter transmits the results of safety inspections conducted by NRC inspectors at your
James A. FitzPatrick reactor facility from October 5 through November 22,1998,
i
Challenges during the period included: problems associated with the implementation of the
L
torus temporary drain rig, the stuck reactor vessel head, and an unplanned exposure event.

Once identified, these challenges were appropriately resolved by your organization.
l~
However, a review of these issues indicates weaknesses in planning and implementation of
technical details.
]
Additionally, based on the results of this !nspection, the NRC has determined that a
l-
, violation of NRC requirements occurred. The violation is cited in the enclosed Notice of
. Violation and the circumstances surrounding it are described in detail in the subject
inspection report. The violation concerned a design deficiency that resulted in the main
steam safety relief valve pilot solenoid cables not being installed in conduit. The failure to
i
install the cables in conduit was contrary to the Final Safety Analysis Report.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and its enclosures will be placed in the NRC Public Document Room. Should you have
