Gentlemen:

SUBJECT: INSPECTION NOS. 50-245/85-30; 50-336/85-35

Enclosed is the report of the team inspection conducted by Mr. R. C. Wilson

and other NRC representatives on November 18 to 22, 1985, at your engineering

offices in Berlin, Connecticut and at the Millstone Nuclear Power Station of

activities authorized by NRC License Nos. DPR-21 and DPR-65. The team's

findings were discussed with you and members of your staff at the conclusion

of the inspection. The inspection reviewed your implementation of a program

as required by 10 CFR 50.49 for establishing and maintaining the qualificati,n

of electric equipment within the scope of 10 CFR 50.49. Within these areas,

the inspection consisted of examinations of selected procedures and records,

interviews with personnel, and observations by the inspectors.

The inspection determined that you have implemented a program to meet the

requirements of 10 CFR 50.49 except for certain deficiencies identified in

the enclosed inspection report. Three of the deficiencies, sumarized in

Appendix A, are classified as Potential Enforcement / Unresolved Items and will

be referred to the NRC Region I office for further action. The most serious

deficiency involves failure to document qualification for internal wiring ir

Limitorque valve operators. The second Potential Enforcement / Unresolved 'eem

involves improper control of revisions to EQ documents and the third, presently

unresolved, concerns a possibly unqualified terminal block in a Limitor que

operator. Five additional concerns are classified as Open Items, and a future

NRC inspection will review your actions concerning them. Details of all the

deficiencies and concerns are discussed in the inspection report.

Your corrective actions regarding the identified deficiencies and concerns

'should not be delayed pending either a future NRC inspection or further

action by the NRC Region I Office.

APPENDIX A

Potential Enforcement / Unresolved Items

As a result of the special equipment qualification inspection on November 18

to 22, 1985, the following items have been referred to NRC Region I as Potential

Enforcement / Unresolved Items (paragraph references are to detailed portions

of the inspection report).

1. Contrary to paragraphs (f) and (k) of 10 CFR 50.49 and sections 5.2.2,

5.2.4 and 7.0 of the D0R Guidelines, at the time of the inspection Northeast

Nuclear Energy Company (NNECO) did not adequately demonstrate qualification

of internal wiring in Limitorque valve operators. (Paragraph 4.D(1),

Item 50-245/85-30-1; 50-336/85-35-1).

2. Contrary to 10 CFR Part 50, Appendix B, Criterion III, at the time of the

inspection NNEC0 did not adequately apply procedural control to the

revision of equipment qualification documents. (Paragraph 4.A, Item 50-245/

85-30-2; 50-336/85-35-1).

3. Contrary to paragraphs (f) and (k) of 10 CFR 50.49 and sections 5.2.2,

5.2.4, and 7.0 of the D0R Guidelines, at the time of the inspection NNECO

did not adequately demonstrate qualification of a terminal block in

Limitorque operator MS-5 of Unit 1. (Paragraph 4.A. Item 50-245/85-30-3).
