-Gentlemen:

We hereby request that you review the attached allegations received by NRC that relate to your operations.. As part of your review, we furthermore

. request that you conduct whatever inspections or investigations necessary to reasonably prove or disprove the allegations. .Should your review of the allegations bring into question a safety issue (such as the adequacy of a safety component, system, or analysis), we expect that that issue will be promptly brought to our attention.

A substantiated allegation may warrant corrective action, and we would expect you to'be prompt and diligent in implementing such corrective measures.

Therefore, we request for the following two potentially safety related allegations that you periodically inform NRC of the resolution of these matters and make records of your completed action available for NRC inspection. It is alleged that:

1.- ~There was cracking of the cavity liner plates. The alleger recommended

that the cracking problem be looked at generically based on lot and heat number.
2. ~Two or three five gallon cans of trash were not removed prior to concrete placement in the Unit I containment wall'at a location 50 feet from the L- personnel airlock in the WNW wall and at the same elevation as the personnel airlock.

The following allegations are forwarded to you for your information.

.1. There'is spider web cracking of the main condenser tube sheets.

'2. The radius and taper of the Squaw Creek Reservoir is not in accordance

. with design. The fill for the dam was made in some _ cases in 18-inch lifts vice the 6-inch lift opecified by procedure. .(Region IV is concurrently informing'the State of Texas on this issue.).

3. The main circulating water pump intake baffles were improperly fitup (gap) and the proper weld procedures were not followed. In addition, the

" emergency' gates" retaining insert for the main circulating water pump is not properly attached structurally. _
