SUBJECT: NRC SPECIAL INSPECTION REPORT NOS. 50 327/97-13 AND 50 328/97 13
Dear Mr. Kingsley:
This refers to the special inspection conducted on July 25 through
September 4, 1997, at the Sequoyah facility.
The purpose of the inspection
was to review aspects of the misalignment of the spare vital battery to the
- 4 vital battery board on July 24, 1997.
The enclosed report presents the
results of this inspection. The exit meeting for this inspection was
conducted on September 4, 1997, with Sequoyah senior management,
Based on the results of this inspection, four apparent violations were
identified and are being considered for escalated enforcement action in
accordance with the " General Statement of Policy and Procedures for NRC
--
Enforcement Actions" (Enforcement Policy) -NUREG 1600.
The four violations-
are related to the mispositioning of a breaker which resulted in not properly
aligning the #5 vital battery to the #4 vital board for a period of
approximately 30.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />. The apparent violations include:
failure to comply
with the required Technical Specification (TS) action statement for an
inoperable vital battery channel; failure to follow procedures and/or
inadequate procedures associated with independent verification and Auxiliary
Unit Operator (AU0)-rounds: failure to follow procedures associated with
realigning the spare battery to the vital battery board ar.J failure to stop
the evolution.when a procedural step could not be performed; and a failure to
promptly identify an adverse condition (out-of tolerance battery voltage).
The apparent violations remain under NRC review.
After completion of this
review, the NRC will contact you to schedule an open predecisional enforcement
conference, Accordingly, no Notice of Violation is presently being issued for

in addition, please be advised that the number and
characterization of the apparent violations described in the enclosed
inspection report may change as a result of further NRC review.
No response
regarding the apparent violations is required at this time.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of
this letter and its enclosure will be placed in the NRC Public Document Room.
