AMENDMENT OF LICENSE TO ALLOW A FIVE YEAR LICENSE EXTENSION (TAC NO. L31133)
Dear Mr. Fliszar:
In accordance with your letter dated November 19,1998 and pursuant to Part 70 to Title 10 of the Code of Federal Regulations, Materials License SNM-561 is hereby amended to allow a five year license extension. Accordingly, Condition 4 has been revised to inciude the date of January 31,2004, and Condition 10 has been revised to include the date of November 19,
-1996 All other conditions of this license shall remain the same.
- l Enclosed are copies of the revised Materials License SNM-561 and the Safety Evaluation Report, which includes the Categorical Exclusion determination.
