Inspection 50-423/86-12
This refers to the operational team inspection, led by Mr. T. Elsasser of this of-
fice on April 14-18, 1986 at Millstone Unit 3, Waterford, Connecticut, of activi-
ties authorized by NRC License No. NPF-49 and to the discussions of our findings
held by Mr. T. Elsasser with Mr. J. Kelly of your staff at the conclusion of this
inspection.
Areas examined during this inspection are described in the NRC Region I Inspection
Report which is enclosed with this letter.
The purpose of this inspection was to assess the facility's operational effective-
ness, as it moves from the startup test phase to normal operations, in relation
to operational, maintenance, surveillance, quality assurance, fire protection, and
management activities.
This was accomplished by reviewing administrative programs
and procedures, interviewing personnel, touring the facility, and observing in-
process activities.
Based on the results of this inspection, no violations were identified.
Several
findings have been characterized as program strengths or weaknesses and are dis-
cussed in detail in the enclosed Inspection Report (50-423/86-12).
In particular,
two issues raised during the inspection warrant further licensee attention.
First,
we are concerned that the high number of energized control room annunciators at
Unit 3 could adversely impact the control room operator's ability to respond ade-
quately to upset or transient conditions.
Secondly, two Service Water valves,
suspected of having lining detachment problems similar to that of other rubber-
lined valves, have not been inspected for acceptability.
(Tetails of our concerns
are contained in Paragraphs 3.1 and 3.7, respectively, in the enclosed report.
We request that you address these two concerns with respect to their impact on the
safety of plant operations and respond within thirty (30) days of receipt of this
letter, stating actions taken or planned, as well as providing any applicable
analysis.
In general, we have concluded that adequate management controls have been estab-
lished and implemented to support full power operation.
