Dr. J. Nelson Grace SPR 85-014 Page Two September 27, 1985 EVENT DESCRIPTION On September 14, 1985, Main Steam Line Process Radiation Monitor RMG-19A was declared inoperable due to erratic indication.

A Maintenance Work Request was initiated for the repair of the unit.

Health Physics was notified of the inoperable monitor and a preplanned alternate method of monitoring was initiated as required by Action 30, Table 3.3-6 of Technical Specification 3 3.3, " Radiation Monitoring Instrumentation." The cause of the erratic indication was found to be a bad detector.

During the investigation of the erratic indication, it was noted by maintenance personnel that the insulation on the cable which connects the output of the detector to a junction box had dried and cracked due to heat.

This condition resulted in the exposure of the internal shielding which could eventually affect operability of the unit.

The subject cable on process monitors RMG-19B and 19C were inspected and also found to exhibit a similar condition.

Although this condition was not the cause of the erratic indication, a Nonconformance Notice (NCN) was written documenting the as-found condition.

This condition was evaluated and found not to be reportable under the requirements of 10CFR21.

The existing cable (designed for operating temperatures of 60*C) was replaced with a quality related cable rated for operating temperatures of 90aC.

Upon replacement of the detector and associated signal cable, RMG-19A was satisfactorily tested and declared operable September 26, 1985.

The signal cable associated with RMG-19C has also been replaced and the RMG-19B signal cable will be replaced by October 4,

1985.
