The purpose of this inspection was to perform a technical evaluation of your equipment qualification test activities for safety-related equipment and to verify continued implementation of your Quality Assurance Program. Areas examined during the inspectica and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.
During the inspection it was found that the implementation of your QA program failed to meet certain NRC requirements. The nonconformances listed in Appendix A are a result of personnel not following the requirements of Patel's Quality Assurance Manual / Quality Engineering Procedures in revising technical documents, procurement of services and compilation of final assessment reports.
The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.
