Dear Mr. Grimaldi:
This letter is in reference to the contamination event which occurred at your facility on October 29,1997, ar.d was reported to the NRC, Region I office by Mr. John Feeney of the State of New Jersey, Department of Environmentel Protect.lon (NJDEP) at approximately 10:30 a.m. on October 30,1997. Mr. Feeney indicated that a patient of
~ the Brisbane facility had broken an exit sign containing approximately 12-curies of tritium, the day before. In addition, he indicated that personnel at the Brisbane facility were also i
attempting to (ocate two other signs which were unaccounted for. The damaged sign was manufactured by Self Powered Lighting, Inc., of Elmsford, New York and was distributed as a generally licensed device. When the sigri was manufactured it contained five glass tubes conta;ning 5 curies each for a total 25-curies of tritium. He also reported that three of the five tubes had been damaged.
In response to the notification, two NRC inspectors and a Branch Chief were dispatched to your facility in order to gather additicaa. Information, evaluate the condnions associated
.with the breakage of the sign, and obtain wipe samples from various locations at the Brisbane facility including Cottage B (where the incident took place), maintenance shed where the damaged sign was stored, maintenance shop, and the Mule Two (small utility truck) used to transport the broken tritium exit sign. Once on site, the NRC representatives met with you, members of the Department of Human Services 1DHS), representatives of the NJDEP.and Envirogenics, consultants to the DHS. The NRC was informed that the damaged exit sign had been triple-bagged and moved fron, Coitage B, where the incident occurred, to a maintenance shop and subsequently to a maintenance shed located adjacent
' to the maintenance shop. In addition, all occupants'of Cottage B had been relocated,

The NRC attended a briefing which was conducted by Mr. Feeney Upon completion of the briefing the NRC attended a meeting conducted by you for Brisbane rtoff members. Upon completion oi this meetirg, members of the NRC proceeded to obtain wipe samples from the Mule Two vehicle, the maintenance shop, maintenance shed, and Cottage B. The wipes were analyzed by the NRC Region i office. The results of our analysis are attached for your review (Enclosure 1). Results indicate that the maximum contamination occurred in the room where the sign had been demaged.
Urine bioassiya were performed by the U.S. Department of Energy, (DOE) Princeton Group.
Results indicate that ths highest dose received by an individual was 1.67E+01 (16.7 millirem). This dose wcs received by the patient who was responsible for breaking the exit sign. 10 CFR 20.1301 requires that the total effective dose equivalent to individual membora of the public from licensed operations does not exceed 0.1 rom (1 mill;slovert or 100 millirem)in a year. The dose received by the patient is substantially lower than that limit. Follow up urine bloassays have been performed by DOE of those individuals who had the highest initial uptakes. The follow up bioassay results indicate a substantial decrease in tritium content in the urine.
We received a copy of your November 12,1997 letter from NJDEP. This letter encloses urine bloassay results, follow up urine bloassay results, wipe test results and follow-up wipe test results. A copy of this letter is attached (Enclosure 2). In this letter, NJDEP concludes that contammation at the Brisbane facility had been removed. All areas have been released since the wipe analysis indicates that the amount of remaining activity is less than 1000 disintegrations per minute per 100 square centimeters.
We understand that Drisbene employees have completed a search for all exit signs containing tritium in the fecilities possession. All the signs and associated tubes had been accounted for as of November 3,1997. In addition, all these signs have been removed from various locations throughout the facility and secured. Replacement signs which do not contain tritium have been put into place.
On November 20,1997, the two NRC inspectors returned to your facility and performed additional wipes. A total of seven wipes were obtained from various locations within the facility. The wipes were analyzed by the NRC Region I office. The analysis shows that the contamination has been reduced significantly and does not exceed allowable lirnits. These results are also attached for your review (Enclosure 3). In addition, a meeting was held with you and other members of the Brisbane staff in which you indicated that you were in possession of approximately 857,55-gallor drums of radioactive contaminated waste.
On December 4,1997, Mr. Robert Bellam, Chief, Bureau of Er?vironmental/ Safety Compliance, State of New Jerset, informed this office that you properly disposed of all the drums containing radioactive contaminated waste.

Enclosed are copies of Princeton Plasma Physics Laboratories reports on the results of the close out verification wipe test analysis (Enclosures 1,2 & 3) and bio assay analysis (Enclosures 4, 5,6 & 7) collected as a result of the radioactive EXIT sign incident. -
Verification wipe samples were analyzed to demonstrate compliance with the U.S.
Nuclear Regulatory Commissions (NRCs)" Acceptable Surface Contamination Levels" for tritium 2
2 (113) of 1,000 dpnV100cm as per Regulatory Guide 1.86. The wipe tests were taken of 300cm areas to ensure that each area was adequately sampled and to also increase detection sensitivity..
To normalize the results to dpm/100cm', divide the reported dpm value by 3 (e.g.,
2 600dpm/300cm' 200dpm/100cm ). Please note, that wipe sample (Enclosure 1, sample #1) exceeded the acceptable level. Ilowever, after additional decontamination, a follow-up wipe sample (Enclosure 2, sample #1) demonstrated that the area was ecceptable. Also, the wipe sample (Enclosure 3, sample #8) exceeded the acceptable level. This area was also decontaminated and 3 additional wipe samples were taken of the area and the results are listed (Enclosure 3; sample #s 36,37 and 38). All three results were within the NRCs guidelines of 2
1,000 dpnV100cm. Therefore, all final verification wipe test results were in compliance with NRCs " Acceptable Surface Contamination Levels" The bio assay results revealed that only three people received radiation doses that exceeded I millirem (i e., mrem) as a result of the incident. Please share these results with your physicians and your staff.
