Description:==

While dispositioning Requirement SWP-0059, the following discrepancy was discovered:

FSAR Section 9.2.1.2 states that the SW Pumps [3SWP*P1 A, B.

C, and D] shall be capable of operating without lubricating water.

Contrary to the above, the procurement specification,2332.502-004, Rev.1, Page 1 12 states:

" bearings shall be lubricated with filtered seawater fumished from the pump discharge header having a maximum particle size of 50 microns" and "In case of loss of power, the purnp shall be capable to siart without lubricating water after a momentary shutdown of 35 seconds until emergency powest is restored."

Further, the data supplied by the manufacturer in respense to the specification states:

" Lubricating water required - gpm/psig: = Positive flow - 30 psig 100 microns" Finally, the manufacturer's drawing [ Hayward -Tyler Dwg No 01-400 520] indicates:

" Filtered seawater to be supplied to gland and bearings 30 seconds max after startup and maintained continuously at 0.5 to 1.0 gpm quantity at 100 microns."

From this it is concluded that the pumps as fumished did not meet the original specification requirement that the pump be able to start up and run for at least 35 seconds without tube l

water, and further that the pumps are not in compliance with the FSAR requirement that they be able to run without lube water at all.

12/22/97 INVAUo: Subsequent to initiating this DR, it was found that modifications had been implemented to the Service Water Pumps, 3SWP*P1 A, B, C, &D which changed out the original bearings, which required lubricating water, with bearings of an improved design, which no longer require lubricating water from an extemal source, but rather are lubricated by the process fluid.

Description:==

There is a discrepancy between the FSAR and the SWS DBSD witt: regard to radiation monitoring requirements. FSAR Sections 9.2.1.2 and 9.2.1.3 require that one monitor per train be provided, or 2 total, to detect radioactivity in the SW discharge from the Containment Recirculation Coolers, while the DBSD Section 12.6.1 requires one monitor per cooler, or 4 total.

Sufficient valving is provided to enabla the operator to remotely isolate one cooler at a time in the event of a hl rad alarm So that the individual affected cooler could be isolated if needed.

Calculations: SP-3SWP-8, Rev 1, CCN#1; SP 3SWP 7. Rev 1, CCN#1; SP 3SWP-8, Rev 1, CCN#1; SP 3SWP 9, Rev 1 CCN#1; SP 3SWP 11, Rev 1, CCN1; SP 3SWP 12, Rev 1, CCN#1; SP 3SWP 15, Rev 1.

All of the set-point calculations identified above are for thermal relief valves downstream of heat exchsngers in the service water system. In the event that service water becomes isolated in one of the heat exchangers while hot process water is flowing through the other side of the heat exchanger, the relief valve may be required to vent if the pressure exceeds the set-point pressure. Calculations to determine tM muired relieving capacity of the relief valves were not lareced in the set point calculations, nor were such calculations entified in the course of the review. Because there is no information referenced that demonstrates the relief valves have sufficient capacity to fullfull their safety related function, this portion of the discrepancy affects system functionality.

. Discrepancy: Closure of Design Deficiency Report (DDR) No. 641

Description:==

Discussion DDR 641 was written to document a concem regarding the potential for trapped air in the RSS system being delivered to the suction of the operating Charging and Safety injection pumps.

Closure of the DDR reli9d in changes to Operating Procedure EOP 35 ES 1.3 to address the issue. These changes were incorporated into EOP 35 ES-1.3 Rev.1 Change i per PORC meeting number 3-86-14. Later, Revision 4 of EOP 35 ES-1.3 removed the changes that had been made without a safety evaluation per PORC meeting No. 3-9126. The lack of safety evaluation to remove the changes (which had been made to resolve DDR 641) has been documented by NU in CR M3 1260. The corrective action for this CR is to " determine what changes, if any, should be incorporated into ES 1.3 after RSS modifications are comple:a to the system."

RFI 652 requested a description of specific changes which were to be made to ES1.3 resulting from CR M3-971260. NU's response (sent via IRF 874) states that "the changes were originally made to the procedure in order to resolve the concem with air intrusion into the operating SlH and CHS pumps following an accident. This concem is now being addressed by permanent plant modification DCR M3 97-045."

The executive summary and supporting design change package details for DCR M3-97045 Rev. C were submitted by NU as attachments to IRF 874 and have been reviewed. Contrary to the IRF response that DDR 641 issues (air intrushn in SlH and CHS pumps) were now being addressed by permanent plant mod M3-

97045,
1. There is no description in DCR M3-97045 which restates the problems and specific concems which are documented in DDR 641. Based on our review of the documentation provided, it is not clear that the concems in DDR 641 are being addressed in the modification.
2. DDR 641 is not listed as a design input or reference
3. The DCR contains listings of various documents which are being addressed by the modifications, these include a listing of ACR's addressed, a listing of UIR's addressed, a listing of OIRs addressed and LERs addressed. DDR 641 is not listed among those documented issues being addressed by the modification.

Background:

Page 8.3 21 of the FSAR states:

The second level of pro 2ection is in addition to the undervoltage scheme which also employs a two-out-of-four coinc!dence logic to prevent spurious trips of the offsite power source. Two separate time delays are incorporated in the degraded voltage scheme. The first time delay establishes the existence of a sustained degraded voltage on the bus. Following the delay, an alarm in the control room alerts the operator to the degraded condition. The subsequent occurrence of an accident signal (SIS or CDA) immediately separates the Class 1E distribution system from the offsite power system..

Discussion:

When the first time delay (relay 62H-) times out and an accident condition is present, relay 27Y2 and timer 62Y are energized. A contact from 27Y2 trips the bus tie breaker and a time delay closed contact from 62Y-energizes relay 27Y1. The reserve breaker is then tripped by a contact from 27Y1.

Based on calculation NL-042, revision 2, timer 62Y-is set to time out at 5 seconds.

Conclusion:==

The FSAR states that the Class 1E distribution system is immediately separated from the offsite power system in the event of an accident signal and a degraded voltage condition.

Based on a review of the schematics it appears that there is a 5 second delay between the time a degraded voltage condition is proven coincident with an accident condition and the Class 1E distribution system is separated from the offsite power system.

Deecription: Design Deficiency Report (DDR) 973 is resolved through E&DCR N-EC-01733. This E&DCR adjusts the setpoint of flow switches without revising the setpoint calculation. The E&DCR does not state what is the average flow value or where this information is documented, The E&DCR states the revised setting is "just below" the average flow value without quantifying this margin.

Discrepancy: CPble and Motor Output Sizing Concems Ir.volving HVAC Motors Ducription: 1. Calculation 067E (CCN 4, Rev 0) establishes maximum cable lengths for 460V motors. The cable size for the feed to 3HVR*ACU1B (Cable # 3HVRBPLOO1, Mart # NHR 25) is not listed in Calculation 067E, as this size of cable (#8 AM) is too small for a 50 HP motor and would lead to an unacceptable voltage drop. Since this cable size is not evaluated, the voltage at the damper motor cannot be confirmed (the results are indeterminate).

2. Specification 2474.030-624 (Rev.1) does not provide brake horsepower load requirements for 3HVR*RE10A,3HVR*RE10B, 3HVR*RE19A, or 3HVR*RE198, therefore, a mathematical evaluation cannot be performed on the data to determine if the licensing requirement (that the motor be sized to develop sufficient horsepower to drive the mechanical load under runout or maximum expected flow anel pressure whichever is greater, and without encroaching on the service factor)is met. (A review of vendor drawings was made, but this data could not be found.)
3. In Specifications 2472.900 594 (Add,1, Rev.1) and 2170.430-140 (Add.1, Rev. 2), dampers are not sized in terms of brake horsepower but are sized to develop torque sufficient to drive the dampers. Specification 2472.900-594 Page A1 13 requires that the torque developed by the motors be at least 1.5 times the torque required by the dampers. However,in Specification 2472.900-594 Pages A2-104 and A2105 and Vendor Drawing 2472.900 5941578, the motors for Dampers 3HVP* MOD 26A and 3HVP* MOD 26B develop 1523 inch. pounds which is only 1.26 times the torque required by the dampers (1206 inch-pounds). No calculation could be found which justifies tnis deviation from Specification 2472.900 594.

Calculation 426CA determines the setting of miscellaneous undervoltage and time dela/ relays associated with the fast and residual voltage bus transfer schemes. One relay,62R, blocks a fast bus transfer if it is delayed for some reason.

The calculation of the setting of time delay relay 62R includes a 1 cycle allowance for the delay due to an interposed auxiliary relay. However, an examination of schematic diagrams 12179-ESK 7J, Revision 20 and 12179-ESK 5BD, Revision 24 Indicates that there is no auxiliary relay that should be included in the setting calculation. After relay 62R times out, the fast transfer is blocked. The omission of the auxiliary relay shortens the time delay for relay 62R to operate and increases the chance that a fast bus transfer would be blocked.

12/29/97 INVALID: This DR was started to report an unanalyzed condition regarding the passive failure design for the QSS/RSS spray headers: that the post-LOCA containment response had not been analyzed for spray flow lost through a crack in one of the spray headers.

This DR should be invalidated because a more careful review of the licensing basis determined that operation of the QSS/RSS piping systems after a LOCA does not require that a passive failure be considered. The QSS/RSS passive failure design basis is that systems that support safe shutdown must be able to perform their safe shutdown function in the event of a passive failure See the disposition of system requirements REQ-MP3-QSS-0001,0078,0310,0359,0433,0746,0747, & 0748,0796,

Discrepancy: FSAR Section 9.5.4 EDG Fuel Oil Storage and Transfer System Desenption Descriptkm: FSAR Section 9.5.4 Item 2 states that the primary fuel oil transfer pump discharge strainer is 0.062 inch perforation size (1575 microns).

Based on ACR M3 96-0240, the primary feed pump has a 70 micron strainer on hie pump discharge.
