NOTIFICATION OF 10CFR PART 21 ARIZONA NUCLEAR POWER PROJECT FVNGS UNIT 1, 2, 3 I.
Condition Description While reworking a loose wire in BOP /ESFAS cabinet "A",
in Unit 2, the engineer and technician determined that the individual module
" multi-nate" pin connectors, manufactured by the AMP Corporation and supplied by Ceneral Atomic, may not meet the required contact length.
An investigation, conducted on April 29, 1986, determined that this was a common problem to both of the Unit 2 BOP /ESFAS cabinets.
Units 1 and 3 BOP /ESFAS cabinets also have similar pin connnectors.
The subject pin connectors number 9 per train and 18 per unit.
The part numbers are P/N 200276-2 and P/N 200277-2.
The configuration within the BOP /ESFAS cabinets has the potential for causing safety-related equipment dependent upon ESFAS actuation to fail to start. The postulated failure mechanism could result in both Diesel Generators being unavailable to supply power to essential safety functions.
II.
Approach To and Status of Proposed Resolution An engineering evaluation is presently underway to address this issue and incorporate it into DER 86-19, which deals with other concerns in the BOF/ESFAS cabinets.
The vendor has been informed of the present condition.
III. Projected Completion of Corrective Action And Submittal of the Final Report The complete evaluation and final report are forecast for May 23, 1986.
