SUBJECT: NRC INSPECTION REPORT 50-298/98-07 AND NOTICE OF VIOLATION
Dear Mr. Horn:
From October 4 through November 14,1998, inspection was conducted at your Cooper Nuclear
Station reactor facility. The enclosed report presents the scope and results of that inspection.
During this inspection, two violations of NRC requirements were identified. The first violation
involved the use of standing orders to direct operator actions such that, in effect, procedure
changes were being implemented without processing the changes in accordance with Technical
Specifications requirements. The second violation involved the cycling of six torus-to-drywell
vacuum breakers prior to obtaining their as-found lift values. This practice preconditioned the
';
vacuum breakers, rendering the as-found test results invalid.
The NRC has concluded that information regarding the reason for the violations and the
corrective actions taken and planned to correct the violations and prevent recurrence are
already adequately addressed in this inspection report. Therefore, you are not required to
respond to this letter unless the description therein does not accurately reflect your corrective
actions or your position in that car;e, or if you choose to provide additionalinformation, you
should follow the instructions specified in the enclosed Notice.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosures and your response wiil be placed in the NRC Public Document Room.
