o DUKE POWER COMPANY CATAWBA NUCLEAR STATION RESPONSE TO VIOLATION NO. 413/86-05-04 Violation Technical Specification 6.8.1, requires that written procedures shall be established, implemented, and maintained covering the activities recommended in Appendia a of Regulatory Guide 1.33, Revision 2, February 1978. Appendix A of Regulatory Guide 1.33, Revision 2, February 1978, requires that maintenance that can affect the performance of safety-related equipment should be properly pre planned and performed in accordance with written procedures, documented instructions, or drawings appropriate to the circumstances.
Contrary to the above the following maintenance procedures and instructions that can af fect the performance of safety-related equipment were inadequate.
(1)
IP/0/A/3820/01, Limitorque Valve Operator Corrective Maintenance, step 10.14 incorrectly. required the use of Unirex N2 grease in the gear case of Limitorque valve operators SMB-000 through SMB-4t.
This error created the potential for mixing potentially incompatible greases in safety-related valves operators since other procedures require the proper use of Nebula EPO grease.
(2) The Catawba Lubrication Manual was inadequate in that it failed to specify what type of lubricant is required for Limitorque valve operators, Models SMB-00 and SMB-000 with serial numbers below #295810 and Models SMBO, SMB1, SMB2, SMB3 and SMB4 with serial numbers below
- 302262. Various maintenance procedures refer to the Lubrication Manual for selection of proper lubricant including MP/0/A/7600/55, Limitorque Valve Actuator Type SMB-000 Corrective Maintenance and MP/0/A/7600/58, Limitorque Valve Actuator Type SMB-00 Corrective Maintenance.
Response (Item 1) 1)
Duke Power Campany admits the violation as stated.
2)
The lubricant specified in the procedure was a QA-approved lubricant available on-site and therefore was thought to have been acceptable for the applications.
3)
IP/0/A/3820/01 was revised, deleting the lubricant type and referencing the Catawba Lubrication Manual to specify the correct lubricant. Also, a search of the Nuclear Maintenance Data Base for work requests involving Limitorque operators and IP/0/A/3820/01 revealed no situations where lubricants were mixed on safety-related actuators.
4)
The corrective action taken in 3) above will avoid further violations.
5)
We have been in full compliance since January 22, 1986.

Duke Power admits this part of the violation as stated.
2)
The Catawba Lubrication Manual failed to specify a lubricant type for the serial numbered models mentioned.
3)
The Catawba Lubrication Manual was revised on January 20, 1986 to correct the discrepancy.
4)
The revised Catawba Lubrication Manual should prevent an accidental addition of the wrong lubricant to the Limitorque Operator. Also, to
, prevent further violations of this type, the Lubrication Manual will be monitored and revised as information (i.e. INPO reports, vendor notices, etc.) becomes available. The Limitorque operators are the only components at Catawba Nuclear Station which have lubrication requirements based on serial numbers.
5)
The revision on January 20, 1986 brought us into full compliance.
