Dear Mr. Shafer:

PRAIRIE ISLAND NUCLEAR GENERATING PLANT Docket No. 50-282, License No. DPR-42 Docket No. 50-306, License No. DPR-60 In response to your letter of April 30, 1984 which transmitted Inspection Reports No. 50-282/84-03 (DPRP) and 50-306/84-03 (DPRP), the following information is provided. In accordance with your letter, the response to the noncompliance identified in Appendix B is submitted as a separate enclosure.

Violation Title 10 of the Code of Federal Regulation; Part 50, Appendix B, Criterion V (10 CFR 50 Appendix B, Criterion V) requires,

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."

Section Work Instruction SWI-0-4, Part 3.6.1, Step 4 requires for the Plant Operations Log,

" Entries which indicate the initiation of an action or the occurrence of some plant condition are not complete entries in themselves, must be noted with an X placed in the left-hand margin adjacent to the entry.

When the referenced action is completed or the plant condition is returned to normal, a log entry will be made. Also, the X alongside the initial entry will be circled and initialed by the operator and the page number of the concluding entry will be noted."

Part 3.6.2 Reactor Logs Step 2 requires, "AlllogentryinstructionslistedforthePlantOperationsLogapply When SepsrwthoRhactor Logs."

ATTACliM5NTS CONTA?N 9 84 SAFEGUARDS !NFORMATidN W. D. Shafer Contrary to the above, at 8:00 a.m., April 6,1984, the Unit 2 high range shield building ventilation radiation monitor, 2R50, was taken out of service entering the action statement of technical specification 3.15.B.1.

On April 9,1984, the 2R50 radiation monitor was returned to service without being logged in the Reactor Log as described above.

This is a Severity Level IV violation (Supplement I).

Response

To improve the logging of equipment status, SWI-0-4 will be revised. The revision will be made, and all operators will review the revision, by July 20, 1984, at which time full compliance will have been achieved.

Since this violation seems to have no more than minor safety or environ-mental significance, we believe an error may have been made in its classifi-cation of Level IV; Level V seems more appropriate.
