PILGRIM INSPECTION REPORT 50 293/97-11 AND NOTICE OF

VIOLATION

Dear Mr. Olivier:

"On November 10,1997, the NRC completed an inspection at the Pilgrim N sclear Power

Station in Plymouth, Mattachusetts. The enclosed report presents the results of that

inspection.

During the eight (8) weeks covered by this inspection period, the conduct of activities at

the Pilgrim facility was characterized by safe plant operations. Operators responded

effectively to two different events. The events involved the partialloss of foed water

-:

heating and ako the unplanned trip of a reactor recirculation pump.

Based on the resu' ' sf this inspection, a violation of NRC requirements was identified

involving two examples of problems with tagging process implementation. The first

example involved three licensed operator errors involved with writing and implementing a

tngout for the backup battery charger breaker, which resulted in tagging the breaker in the

wrong position. We acknowledge the positive actions of an off going operator who

identified this tagging error; however, we chose not_to exercise discretion and cite this

violation based on the multiple errors made and a weakness in your apparent cause

analysis. The NRC also identified a second tagging error involving the adequacy of the

boundary valve tagout for the air start compressor for the "A" emergency diesel generator.

enclosed Notice when preparing your response. In your response, you should document

the specific actions taken and any additional actions you plan to prevent recurrence. Your

response may reference or include previous docketed correspondence if the

correspondence adequately addresses the required response. After reviewing your

response to this Not!ce, including your proposed' corrective actionji and the results of

future inspections, the NRC will determine whether further NRC pnforcement action is

necessary to ensure _ compliance with NRC regulatory requirempts.
