Telephone Conversation between P. Narbut and T. Bradish on May 10, 1984 Dear Sir The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.
Due to the extensive investigation and evaluation required, an Interim Report is attached.
It is now expected that this information will be finalized by August 24, 1984, at which time a complete report will be submitted.

INTERIM REPORT - DER 84-27 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)
PVNGS UNIT 1, 2, & 3
- 1. Potential Probica During subcontrtactor's (The Waldinger Corporation) surveillance walkdown of their field installed instruments it was noted that temperature switch 2-J-ilDA-TSL-17 and 2-J-!!DN-TSIIL-25 control instrument 3-J-IfJA-M57 and 3-J-ilJA-UY-39B were removed, disassembled and/or modified after final inspection and the following conditions were noted:
- 1. Instruments were reassembled incorrectly and relocated without documentation te serve notice to other disciplines that a change was initiated.
- 2. Work control procedures during reinstallation were not adequate to implement all design requirements.
II. Approach To and Status of Proposed Resolution Bechtet engineering is currently working with W41dinger on this problem to detemine reportability and technical justification for corrective actio n.
III. Projected Completion of Correctivo Action and Submittal of the Final Report Evaluation of this condition and submittal of the Finni Report is forecast to be completed by August 24, 1984.
