Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to the extensive investigation and evaluation required, an Interim Report is attached. It is now expected that this information will be finalized by August 15, 1984, at which time a complete report will be submitted.

v INTERIM REPORT - DER 84-26 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS) r PVNGS UNIT 1 I. Potential Problem The plunger interlock of circuit breaker 1E-PBB-SO4D was found with i

a bent "C" shaped clip. In addition, the plunger interlock bolt was found nested too low in its guide bracket and with an excessive number of flat washers installed. This condition is described in NCR SE-4056. A similar condition involving bent "C" clips has been i

identified on NCR SE-3997 (orignially NCR SE-0609).

Discussions with Mr. L. Lowery of APS Breaker Test Group indicate that the bent "C" clips may be due to improper testing and adjustment by jobsite personnel. In order to perform a portion of the breaker test and adjustment procedure, an adjustable test jig must be installed on the breaker interlock operation linkage prior to test closing of the breaker. The problem occurs when a jig for a different type / size of breaker is used (several different size jigs are available on the job-site) or the correct jig is improperly installed / tightened.

When the breaker is closed during a test, the "C" clip is strained and deformed if either an improperly installed test jig, or the wrong size jig is used.

II. Approach To and Status of Proposed Resolution A new NCR is being issued which requires inspection of all Class IE 4.16 kV breakers. This inspection is to determine the extent of plunger interlock maladjustment and "C" shaped clip damage. The results of the inspection and the extent of readjustment or repair needed shall be reported to Bechtel engineering via the NCR for evaluation prior to final disposition of this DER.

III. Projected Completion of Corrective Action and Submittal of the Final Report i

Evaluation of this condition and submittal of the Final Report is forecast to be completed by August 15, 1984.
