NPPD Response to Inspection Report 50-298/88-07 This letter is written in response your letter dated May 11, 1988, transmitting Inspection Report 50-298/88-07. Therein you indicated that one of our activities was in violation of NRC requirements.

Following is a statement of the viclation and our response in accordance with 10CFR2.201.

Statement of Violation Failure to Follow Health Physics Procedure 10 CFR Part 50, Appendix B, Criterion V, and the licensee's approved Quality Assurance program require that activities affecting quality be accomplished in accordance with approved instructions, procedures, and drawings. CNS Procedure 0.4, "Preparation, Review, and Approval of Procedures," Revision 11, dated February 18, 1988, specifies that approved written station procedures shall be adhered to by all station personnel.

CNS Health Physics Procedure 9.1.1.4 "Special Work Permit," (SWP)

Revision 16. dated April 15, 1987 Section VIII.C.4 states, "Each individual must sign in on the Special Work Permit, and in so doing, acknowledges understanding and compliance with all specific requirements nie t forth on the Special work Permit."

Contrary to the above, Health Physics Procedure 9.1.1.4, SWP, Revision 16, dated April 15, 1987 Attachments A and B, were not adhered to by an individual located inside the boundary area of the SWP on March 28, 1988.

The individual did not sign in on SWP 88-3-46, issued on March 21, 1988, and was not wearing all the protective clothing required by the SWP.

This is a Severity Level IV violation. (Supplement I) (298/8807-01)

Reason For the Violation Individual did not adhere to Health Physics Procedure 9.1.1.4, SWP, P.evision 16, dated April 15, 1987, Attachments A and B.

Nuclear Regulatory Commission Page 2 June 7, 1988 Corrective Steps Which Have Been Taken And the Result Achieved The individual involved was verbally reprimanded and counseled by the Maintenance Manager and Maintenance supervisor. A copy of the CNS hadiological Incident and Accident Report was placed in the individual's personnel file at the direction of the Maintenance Manager. The repcrt will remain in the file for a period of at least six months. The individual's performance will be evaluated during this time frame with regard to this incident. Upon completion of the evaluation period, the incident report may only be removed from the file at the Maintenance Manager's direction. This individual's previous performance has not indicated any problems with radiological procedure compliance.

Corrective Steps Which Will Be Taken To Avoid Further Violation Maintenance Department Supervisors have been instructed to discuss this incident with all Maintenance Department personnel. Supervisors are to stress the impcrtance of adhering to station procedures.

Date When Full Compliance Will Be Achieved Full compliance will be achieved by June 15, 1988.
