APPENDIX A NOTICE OF VIOLATION Kansas Gas and Electric Company Docket: 50-482/86-05 Wolf Creek Generating Station License: NPF-42 During an NRC inspection conducted during the period of March 1 - 31, 1986, one violation of NRC requirements was identified. The violation involved the failure to fully document the performance of a surveillance test in accordance with the procedure. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:

Failure to Document the Performance of a Surveillance Test in Accordance with the Procedure Technical Specification 6.8.1 requires that, " Written procedures shall be established, implemented, . . . covering . . . a. The applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978."

Surveillance Procedure, STS GK-001B, Revision 5, " Control Room Emergency Ventilation System Train B Operability Test," has been established and implemented in accordance with the above Technical Specification.

Contrary to the above, on March 20, 1986, the NRC inspector observed STS GK-0018, Revision 5, being performed without Section 3.0, " Initial Conditions," first being signed as completed.

This is a Severity Level V violation (Supplement I.E.) (50-482/8605-01).

Pursuant to the provisions of 10 CFR 2.201, Kansas Gas and Electric Company is hereby required to submit to this Office, within 30 days of the date of the letter transmitting this Notice, a written statenant or explanation in reply, including for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved; (3) the corrective steps which will be taken to avoid further

, violations; and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
