PLANT OPERATIONS The plant continued with its 13th refueling and maintenance outage during the month of February, t On February 26, 1986 at 12:10, there was a report that while disassembling the reactor for the fuel shuffle, a single fuel assembly-was inadvertently moved and was observed resting on top of the core at an angle inside the reactor vessel.

No apparent damage to the assembly was noted which could lead to the release of radioactive materials from the fuel assembly. Access to the containment was restricted to essential personnel until the situation was evaluated further.

The fuel assembly was subsequently removed from the vessel on March -2, 1986 at 19:05.
