Dear Director:

Enclosed please find $150 check for processing of the application for renewal of Special Nuclear Materials License SNM-1426, issued to Saint Francis Hospital November 5,1973, and due to expire October 31, 1978. We still wish to be allowed to possess Plutonium (principle radionuclide Pu-238) up to a limit of 4.2 grams (20 individual sources not to exceed 210 milligrams each) for impantation in humans as a component of the Medtronic Model 9000 nuclear powered cardiac pacemaker for clinical evaluation purposes.

We are following and wish to continue following the conditions stated in the original license and in the amendment dated March 28, 1974.
