NOTICE OF DEMATION AND NOTICE OF VIOLATION

Dear Mr. Penros:

This refers to the inspection conduded on July 2g through September 12,1997, at the

-l

Quad CHies Nuclear Power station. The purpose of the inspection was to determine whethe-

t

activides authortred by the license were conduded safely and in accordance with

,

NRC requirements. During this period routine resident inspections were performed. Based on

j

As inspectors' findings, overall performance at the Quad Clues station was safe. At the

l

conclusion of the inspection, the findings were discussed with those members of your staff

identi6ed in the enclosed report.

!

While overall operator performance was good, operators failed to ensure the performance of

several Technical Spool 6 cation (TS) required surveillances and displayed a lack of rigor in

restoring some degraded equipmerW to a fully operable condition. Mainter,ance activities were

i

conducted in a satisfactory manner in most cases, but were marked by procedure adherence

problems and errors requiring rework. Some addluonal radiation dose was scoumulated as a

result / Engineering personnel failed to establish adequate design basis information and testing

>

requirements for some equipment. Even though fire risk was identifled as being relatively high at

Quad Cities, the inspectors noted an overell insensitivity towards resolution of fire protocuon

>

lasues.

,

Based on the results of this inspection, the NRC has determined that four violations of

'

NRC requirements with multiple examples occurred, in addhion, a devis3on from a commitment

to the NRC was identified. These violations and the deviation are cited in the enclosed Notice of

!

Violation (Notice) and Notice of Deviation (Notice) and the circumstances surrounding them are

described in detail in the attached inspecuon report. The first violation represents a decline in

!

the performance of surveillance testing with five examples of missed Myelliance requirements

'

'

involving the areas of operations, chemistry, maintenance, and engineering. The second

violation involves two examples where inspectors found the acceptance criteria for TS required

surveillance teste to be inadequate. The third violation involves two examples of improp6r

design control related to the 250 volt safety-related battery. The fourth violation was a conoom

i

^ because it involved two examples of failure to follow procedures in the maintenance and fire

protection areas. The deviation is signi6 cant in that it involved a failure to meet a commitment to

i

the NRC that constituted correcove scuon for en equipment problem as documented in a

l
