e Violation Technical Specification 6.4.1 requires the station to be operated and main-tained in accordance with approved procedures.

Contrary to the above, on February 4, 1986, Procedure IP/2/A/305/3C, RPS Channel "C" Calibration and Functional Test was not followed, in that Instrument and Electrical technicians tripped the Channel "D" breakers instead of the Channel "C" breakers as specified by the procedure. These steps require independent verification by the procedure and were signed indicating the actions taken were independently verified to be correct.

This action resulted in a reactor trip from 100 percent power.

This is a Severity Level IV violation (Supplement I).

Response

1) Admission or denial of the alleged violations:

This violation is admitted as written.

2) Reasons for the violation if admitted:

This violation resulted from personnel error, failure of Maintenance personnel to adequately follow approved procedures.

Additional details are contained in LER 50-270/86-02, dated March 6, 1986.

3) Corrective steps which have been taken and the results achieved:

The involved personnel have received verbal reprimands.

The appropriate procedures have been mcuified to further clarify re-quired actions.

Communications for testing personnel has been im-proved by adding a phone in the cable room near the breaker-keyed test switches.

4) Corrective steps which will be taken to avoid further violations:

A) Maintenance (I&E) management will review the subject procedure with appropriate personnel, emphasizing all changes and explaining what is expected of them when running this procedure.

B) The test switches for the breaker test will have independent keys for each switch and will be under administrative control of the Unit Shift Supervisor.

5) Date when full compliance will be achieved:

The corrective actions noted in (4) above will be completed as follows:

A) June 1, 1986 B) December 1, 1987 (Unit 1 already completed).
