RESPONSE TO VIOLATION I
VALVES NOT PROPERLY LOCKED 414/86-18-01, REVISION 1 i
Technical Specification 6.8.1 requires written procedures to be established, implemented, and maintained covering the applicable procedures recommended in Appendix A to Regulatory Guide 1.33, Revision 2.
Various licensee operating procedures require valves to be in specified positions and locked in that position..
Contrary to the above, on April 17, 1986, valves identified as 2SM70, 2SM71, 2SM73, 2SM142, 2CF95, 2CF97, 2CA119, 2CA120, 2BW10, 2WE22, 2ND53, 2NV65, and 2NV76 were not locked as specified in applicable operating procedures.
RESPONSE
1.
Admission or denial of violation:
Duke Power Company admits the violation.
2.
Reasons for violation if admitted:
The method used to secure these valves in their locked l
position is to use a chain and lock to secure the operating handwheel.
Due to the type of operating handle (a straight bar) on most of these valves, it is difficult to secure the valve to prevent it from being repositioned.
A contributing cause is the level of i
attention to detail of the operator at the time the lock and chain are placed.
Valves 2BW10 and 2ND53 are circular type handwheels.
Valve 2BW10 was not required j
to be properly locked at the time the inspector observed j
the valve due to it being tagged as part of an equipment isolation and the chain and lock was removed to verify i
tagged position. (see attached R&R) l 4
3.
Corrective actions taken and results achieved:
All valves were inspected by shift personnel when the inspector identified these valves.
All valves were in the proper position.
Subsequently, all valves were locked except those covered by R&R's.
4.
Corrective actions to be taken to avoid further I
violations:
A.
A new method of securing the valves with straight bar type handwheels is being investigated.
B.
Additional emphasis on properly locking valves is being placed by discussion of this violation with all shifts at a shift Supervisor's meeting.
- Also, in a requal segment of operator training, a topic of the philosophy on locking / locked valves will be i
included.

The Design Group is reviewing criteria for locking valves.
