NRC INSPECTION AND NOTICE OF VIOLATION
Dear Mr. Carrato:
This refers to the NRC inspection of the activities of Alfred Benesch & Company which was conducted on June 16,1999, at a temporary job site located in the vicinity of Fond-Du-Lac, Wisconsin. The purpose of the inspection was to determine whether activities authorized by the license were conducted safely and in accordance with NRC requirements. At the conclusion of
. the site inspection, preliminary findings were discussed with a member of your staff. Continuing NRC review was performed through June 23,1999, to further evaluate your training and dosimetry requirements. At the conclusion of the inspection, findings were discussed with you via telephone.
During the inspection period, your conduct of licensed activities was generally characterized by safety-conscious gauge operations. We are concemed, however, about two violations of NRC l requirements that were identified by the NRC inspector. The violations consist of: (1) an Individual working with licensed material without having received training from an authorized training program; and (2) failure of the same individual as above to wear dosimetry while working with licensed material.
The violations are cited in the enclosed Notice of Violation (Notice). Please note that you are required to respond to this letter and should follow the instructions specified in the
. enclosed Notice when preparing your response. For your consideration and convenience, an excerpt from NRC Information Notice 96-28," SUGGESTED GUIDANCE RELATING TO DEVELOPMENT AND IMPLEMENTATION OF CORRECTIVE ACTION,"is enclosed. The NRC will use your response, in part, to determine whether further enforcement action is necessary to ensure compliance with regulatory requirements.
