Gentlemen:
This refers to the routine safety inspection conducted by NRC inspectors
M. M. Holzmer and P. L. Eng of this office from April 14 through June 3,1988,
of activities at the Zion Nuclear Generating Station, authorized by Licenses
No. DPR-39 and No. DPR-48, and to the discussion of our findings with
Mr. G. Plimi at the conclusion of the inspection.
The enclosed copy of our inspection report identifies areas examined during
the inspection. Within these areas, the inspection consisted of a selective
examination of procedures and representative records, observations, and
interviews with personnel.
During this inspection, certain of your activities appeared to be in violation
of NRC requirements, as described in the enclosed Notice. With respect to
'
item 1.a. the inspection showed that action had been taken to correct the
identified violation and to prevent recurrence. Our understanding of your
corrective actions are described in Paragraph 4 of the enclosed inspection
i
report. Consequently, no reply to the violation is required and we have
no further questions regarding this matter at this time.
Regarding the remaining items, a written response is required.
This report identifies several findings regarding procedural adherence during
surveillance testing activities, failures to test engineered safeguards logic
in accordance with Technical Specifications, test procedure inadequacies and
test documentation deficiencies. Similar findings in the area of surveillance
testing were identified at Zion in previous inspections during the past year.
We are particularly concerned that the number, type, and repetitive nature of
these findings indicates significant weaknesses in your test programs and poor
application of quality program principles.
Please include in your response to
the enclosed Notice of Violation an expanded discussion of the corrective
actions planned to assure that test activities, documentation, and test
procedures, including temporary procedure changes, are appropriately completed
and maintained in the future to support equipment operability.
In addition,
please include your plans to ensure that engineered safeguards logic tests
are performec in accordance with Technical Specification requirements.
