SUBJECT: NRC INSPECTION REPORT 50-416/98-13 AND NOTICE OF VIOLATION
Dear Mr. Eaton:
From September 20 through October 31,1998, an NRC inspection was conducted at your
Grand Gulf Nuclear Station reactor facility. The enclosed report presents the scope and results
of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the Grand
Gulf facility was generally characterized by safety-conscious operations, sound engineering and
maintenance practices, and careful radiological work controls. However, the inspection

identified two violations consisting of a failure to implement corrective actions following
Refueling Outage 9 for known deficiencies in containment grating and four examples of failure
'
to follow procedures associated with the partial unlatching of a tool ring during a heavy lift over
the recently refueled reactor in May 1998. The second violation is of concern because the
failures to follow procedures posed an increased risk of dropping a heavy load over the reactor
core.
The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are~ described in detailin the enclosed report. The NRC has

concluded that information regarding the reason for the first violation and the corrective actions
taken and planned to correct the violation and prevent recurrence are already adequately
addressed on the docket in the subject NRC Inspection Report 50-416/98-13. Therefore, you

are not required to respond to the first violation unless the description therein does not
accurately reflect your corrective actions or your position. In that case, or if you choose to
provide additional information, you should follow the instructions specified in the enclosed
Notice. Please note that you are required to respond to the second violation and should follow
the instructions specified in the enclosed Notice when preparing your response. This response
is required because the violations were not identified through the investigations you held onsite
and have not been entered into your corrective action program. The NRC will use your
response, in part, to determine whether further enforcement action is necessary to ensure
compliance with regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its

enclosures, and your response will be placed in the NRC Public Document Room (PDR).
