NEH work rquest 95742 had been signed "control accepted" on June 24, 1988 by

distributed to appropriate Operations personnel prior to heatup of the Nactor Cbolant (NC) System.
Instead, Oreraticns atS personnel decided to i
include infomation about NSM MG-22062 with other NSHs that had been l
capleted during this refueling outage into one required reading package, and i
to distribute it at the end of the outage.
On July 23, 1988, Operaticos

personnel proceeded with NC systan heatup to a T-ave of 557 degrees-F without knowledge of the T-ave setpoint change to 553 degrees-F.
