Dear Sir or Madam:

l This letter constitutes a supplement to ARCO's response to the Notice of Violation issued by the Nuclear Regulatory Commission on May 12,1999. The Notice of Violation resulted from the inspection of the Roy F. Weston facility used to store and receive ARCO Nuclear Medical Products Company acemakers after they are explanted from patients. The Notice of Violation referenced a sic ' violation. ARCO 's Nuclear Materials License (SNM 1993) authorizes two Roy F. Wer employees, Richard Shimko and Patrick Rafferty, to store the licensed material (pacemah..s). Richard Shimko is identified as the radiation safety officer.

Both Mr. Shimko and Mr. 4: Eerty left Roy F. Weston's employ during 1998. Weston i

notified ARCO of this fact, but because of an internal oversight, ARCO's license amendment was not submitted to the NRC.-

Per the Reply to Notice of Violation, dated June 10,1999, attached is a copy of the NRC i

Form 313, which has been submitted to amend SNM 1993. The amendment request reflects the replacement of Mr. Shimko and Mr. Rafferty with Laurence F. Smith and Robert Massengill. Both Mr. Smith and Mr. Massengill will receive and store the licensed material. Mr. Massengill will serve as the Radiation Safety Officer. Copies of Mr. Smith
