SUBJECT:

PEACH BOTTOM ATOMIC POWER STATION STATUS REPORT FOR THE PERIOD NOVEMBER 7 TO 28, 1988 Enclosed is the Peach Bottom periodic status report from the NRC Resident Office at Peach Bottom. Three NRC resident inspectors monitored actn :D es at the plant during the report period.

These status reports are intended to provide NRC management and tne public 3

with an overview of plant activities and NRC inspection activities.

Subsequent inspection reports will address many of these topics in more detail.

9 ENCLOSURE PEACH BOTTOM ATOMIC POWER STATION STATUS REPORT FOR THE PERIOD November 7 to 28, 1988 1.0 Plant Status t

As of 8:00 a.m. on November 28, 198?, Units 2 and 3 are in a cold condition per the March 31, 1987, NRC Order with coolant temperatures less than 212 degrees Fahrenheit.

The Unit 3 core is completely off loaded into the spent fuel pool.

2.0 Facility Operations Summary Unit 2 is in day 626 of its seventh refueling outage that began March 13, i

1987. Unit 2 maintenance repair work is in progress on plant systems required for restart. The Unit 2 reactor pressure vessel operational hydrostatic test was successfully completed during the period November 22 to 23, 1988. Unit 3 is in day 425 of its seventh refueling outage that began October 1, 1987.

Unit 3 maintenance and modification work is underway on 33 of 56 plant systems.

3.0 Items of Special Interest 3.1 On November 15, 1988, the Nuclear Regulatory Commission staff cited PEco for numerous violations of security requirements at the Peach Bottom Atomic Power Station. A fine of $50,000.00 was proposed.

3.2 At the request of the licensee, two management meetings with the NRC were held at the regional office. On November 8, 1988, the licensee discussed the radiation protection program and exposure results from the recently completed 'Init 3 pipe r. placement outage. On November 18, 1988, the licensee presented their configuration control management status and program plans.

3.3 On November 16, 1988, the Region I Administrator testified before the Pennsylvania Senate Consumer Protection and Professional Licensure Committee regarding the need for an investiga:,fon into the Peach Bottom Snutdown Order issues.

Representatives of the licensee, the State of Pennsylvania and the public also provided testimony.

4.0 Other Items of Interest 4.1 On November 10, 1988, the licensee made a four hour telephone notification to the NRC regarding an unanalyzed condition found while shut down.

The licensee discovered that numerous water tight seals were missing or degraded in flood protection walls on both units.

.A 4.2 On November 21, 1988, the licensee made a four hour telephone f

notification to the NRC regarding a condition found while shut down which could affect the ability to remove decay heat. A one inch hole was discovered in the high pressure service water pump bay floor.

The licensee determined that the flood protection capability of this area could have been affected.

4.3 The licensee reported that both fire pumps were simultaneously inoperable on November 16, 1988. Written correspondence was subsequently provided in a letter dated Novemt,er 17, 1988.

4.4 During the period November 14 through 21, 1988, the licensee performed repairs on the Unit 3 reactor pressure vessel manway l

access hole covers.

The licensee cut out the cracked covers and installed bolted cover plates.

5.0 NRC Activities During the Period 5.1 Three NRC Resident Inspectors and a regional inspector were assigned to monitor plant activities between November 7 and 28, 1988.

- Reactor Engineer Areas inspected included operational safety, shutdown order commitments, radiation protection, physical security, control room activities and f

demeanor, reportable events, surveillance testing, ou.tage activities, l

maintenance, power ascension program and outstanding items.

5.2 Five specialist inspection reports were received during the period.

Inspection report (50-277/88-35; 50-278/88-35) reviewed surveillance testing and design modifications.

The inspection was conducted from September 26 to 30, 1988. No violations were noted; however, deficiencies were noted in that system operating procedures forced operators to suspend conduct of two of three surveillance tests for which performance was being evaluated.

Inspection report (50-277/88-32; 50-278/88-32) reviewed the licensed operator requalification program performed at the Peach Bottom plant specific simulator in Columbia, Maryland.

The inspection was conducted from August 31 to September 29, 1988.

No violations were observed; however, several weaknesses were identified in the licensed operator requalification program.

PECo has committed to providing additional simulator training to all of the operating crews.

.L Inspection report (50-277/88-37; 50-278/88-37) reviewed radiation safety, including previously identified items, pipe replacement closure, training, exposure controls, outage work control, survey instrumentation and internal exposure controls. The inspection was conducted from September 26 to 30, 1988.

No violations were identified.

Inspection report (50-277/88-38; 50-278/88-38) reviewed the licensee's corrective actions for previously identified Environmental Qualification findings. The inspection was conducted from September 26 to 30, 1988. One violation with four examples of inadequate splices was identified.

Inspection report (50-277/88-41; 50-278/88-41).eviewed the fire protection / prevention program that included a review of corrective actions for two previous violations and a review of actions tcken to resolve other related concerns.

The inspection was conducted from October 3 to 7, 1988. No violations were identified, 5.4 Three ERC research personnel were on site on November 22, 1988, to tour tne peach Bottom facility and to make an entry into the Unit 3 d rywe ll.
