Dear Mr. Konkey:
This refers to your letter dated October 14,1998, notifying our office of your loss of a polonium-210 static control ionizer. The static control ionizer (NRD, Inc. Model P-2031, Serial No. 96781) contained approximately 20 millicuries of polonium-210. On or about October 14, 1998, the static control ionizer was discovered missing. Search efforts to recover the missing static control ionizer have proven unsuccessful. Your investigation to date has concluded that l
the device is misplaced somewhere within your building.
After reviewing this incident, the NRC has determined that a violation of NRC requirements occurred. This violation is cited in the enclosed Notice of Violation. A written response is not required because your report of the loss of the static controlionizer dated October 14,1998, l
identifies action taken by you to prevent future losses.
Although the amount of polonium-210 discarded was small, the violation demonstrates the importance of maintaining adequate control of all radioactive material to ensure that improper handling or disposal does not occur. We caution you that any similar violations in the future may result in additional enforcement action, including the imposition of monetary penalties and/or orders suspending, revoking or modifying your license.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its enclosure and a copy of your notification of loss will be placed in the NRC Public Document Room.
