SUBJECT:
INSPECTION 50-170/88-04
This letter transmits the findings of the routine safety inspection conducted
by Messrs. J. Williams, J. Gadzala and O. Wallace of this office on October 26 -
28 and November 7,1988 at the Armed Forces Radiobiology Research Institute,
Bethesda, Maryland. The inspection consisted of document reviews, interviews,
and observation of activitios. The results were discussed with Mr. M. Moore and
others of your staff on October 28, 1988. Additional findings identified
November 7, 1988, and after leaving the site, were discussed in a subsequent
telephone conversation with Mr. Moore on November 21 and 22, 1988.
Based on our inspection, four apparent violations were identified.
Required
safety reviews were not performed, reactor operations were conducted without
sampling gaseous effluents, one procedure is inadequate to assure safe operation
of the reactor and several written procedures were not being fully adhered to,
and operator requalification training was not being adequately conducted.
These matters will be discussed at an Enforcement Conference at our office in
King of Prussia, Pennsylvania af ter you have had time to review this report.
We will contact you shortly to schedule such a meeting. At that meeting you
should be prepared to discuss these matters, their causal factors, as well as
the corrective measures that you have taken or plan to take to prevent recur-
rence. Our decision concerning enforcement action for these matters will be
communicated to you by separate correspondence following the enforcement con-
ference.
In accoraance with 10 CFR 2.790(a), a copy of this letter and the enclosures
will be placed in the Public Document Room.
