Gentlemen:

SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/88-26 AND 50-328/88-26)

This refers to the special Nuclear Regulatory Commission (NRC) shift

coverage inspection conducted by the NRC's Sequoyah Restart Task Force

on April 3, 1988 thru May 4, 1988. The inspection included a review of

the activities associated with the heatup of Sequoyah Unit 2. At the

conclusion of the inspection, the findings were discussed with those

members of your staff identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report.

These areas included extended control room observation and operational

safety verifation. Within these areas, the inspection consisted of

selective examinations of procedures and representative records,

interviews with personnel, and observation of activities in progress.

The inspection findings indicate that certain activities appeared to ,

violate NRC requirements. Accordingly, the violation, references to l

pertinent requirements, and elements to be included in your response are I

described in the enclosed Notice of Violation. In light of similar

violations previously cited in Inspection Reports 327,328/87-30, 87-66,

and 88-06, the NRC still has concern regarding the overall adequacy of

configuration control at Sequoyah. Please consider this previous

enforcement history in your root cause evaluation of the violation

herein.

Two unresolved items were identified during the inspection. An

unresolved item concerning delays in entering and reporting LCO entry on l
