Dear Sir:
Subject:
Vermont Yankee Response to IE Compliance Bulletin No. 87-02, "Fastener Testing to Determine Conformance with Applicable Material Specifications" By letter dated November 11, 1986 (Reference b), the NRC's Office of Nuclear Reactor Regulation requested that licensees 1) review their receipt inspection requirements and internal controls for fasteners; and 2) indepen-dently determine, through testing, whether fasteners (studs, bolts, cap screws and nuts) in stores at their facilities meet required mechanical and chemical specification requirements.
In accordance with the subject Bulletin 87-02, the following information is previded in response to the six specific actions requested.
ACTION ITEM 1 describes the fastener characteristics examined during receipt inspection and internal controls implemented during storage through end use.
ACTION ITEMS 2 AND 3 As prescribed, eleven (11) safety related fasteners, ten (10) cafety related nuts, ten (10) non-safety related fasteners, and ten (10) non safety related nuts were selected from current, in use, stock.
Attempts were made to select materials representative and proportional to plant use.
In addition, all portions of the selection process were performed in concert with the two Vermont Yankee NRC resident inspectors.

Nuclear Regulatory Commission January 12, 1988 Page 2 ACTION ITEMS 4 AND 5 Chemical, hardness, and mechanical testing of the selected fasteners was performed by a Vermont Yankee approved vendor (Dirats Laboratories) as spe-cified by the subject Bulletin Action Item 4.
Results of all tests "ith sup-porting information is provided in accordance with Bulletin Action item 5 in.
Based on the results of the required testing, the following specimens were identified to have single elements out of prescribed range.

- VY-87-02-13SR-N Carbon 0.05% Lo*
Nut
- VY-87-02-18SR-N Chrome 0.44% Lo*
- Below specified range Based upon the review of the above discrepancies, additional testing was performed on specimens taken from the same (respective) lots. This testing included chemical analysis, hardness testing, and proof testing performed in accordance with the requirements of ASTM A-194.
For those nuts from the same lot as specimen #VY-87-02-13SR-N, all sub-sequent test results were within specified chemistry and hardness ranges.
Proof tests were also performed and the results were satisfactory.
For those nuts from the same lot as specimen #VY-87-02-18SR-N, subsequent test results were identical to the earlier test in that chemistry analysis iden-tified that the chrome content was typically 0.44% low. All other chemistry and hardness was within specified range.
Proof tests were also performed and the results were satisfactory.
Specific results of this subsequent testing is provided in Enclosure 3.
A review and evaluation of the chemical properties, hardness tests plus subsequent proof testing of these specimens concludes that had these items been used, their structural functionality would rat have been impaired and, there-fore, would have had no adverse affect on the operability of safety related plant components.
ACTION ITEM 6 Based on the above testing and supporting evaluation and the review of existing procedures and material controls, no other actions are planned at this time.

Prior to receipt inspection, fasteners are subjected to a cursory inspection to verify these items were not subjected to shipping damage.
This inspec-tion will identify rough handling, excessive exposure, contamination, and other environmental or shipping influences.
B.
The formal Receipt Inspection of safety related fasteners records, verifies, or examines the following characteristics:

Verification of the delivered material agains the Purchase Order:
a.
Purchase order number b.
Item number c.

Part number e.
Serial / heat / batch lot number f.
Drawing / spec, number g.
Quantity received and quantity inspected h.

Part numbers, heat numbers, lot numbers, and/or serial numbers match associated documentation (as required by the Purchase Order).

Certification (as reauired by the Purchase Order) a.
C of C or certified material test reports traceable to Purchase Order and the item.
b.
C of C or documentation that the item was manufactured, inspected, tested, and/or supplied in accordance with the Purchase Order requirements.

Control of safety related fasteners (storage and issuance) is detailed as follows:

Upon successful completion of the receipt inspection, safety class fasteners are identified with a red "P" tag which contains the following information as applicable:
a.
Purchase order number b.
Safety class c.
Stock code and description d.
Vendor / manufacturer part number e.
Serial / heat / lot / batch number f.
Storage location and level g.
Application h.
EQ and shelf life data i.
Inspector's initials and date 2.
At the completion of acceptance of non-safety related fasteners, the fasteners may be identified with a green tag which reflects the following as applicable:
a.
Purchase order number b.
Stock code and oescription c.
Vendor / manufacturer part number d.
Level of storage e.
Inspector's initials and date NOTE:
Non-safety related fasteners may be stored without tags.
Any material without the red "P" tag must be assumed to be non-safety related.
ENCLOSURE 1 (Continued) 3.
Except as required by the material specification, safety related
-fasteners one inch nominal diameter and smaller, and other pro-ducts, where the largest space available for marking is one inch in any direction, need not be individually marked provided that they are stored in packages or containers which are clearly identified by legible marking to insure positive identification of the material. All other fasteners are marked with their purchase order number as follows:
a.
Electrochemical etching may be used on:
1.
Hardened material with minimum required yield strength of 90,000 psi or greater.
2.
Cobalt based materials.
3.
Pressure boundary parts less than 1/8 inch thick, b.
Vibration marking 1.
Materials greater than 1/8 inch thick.
c.
Stamping 1.
Materials greater than 4 inch thick, d.
Ink markers 1.
Ink markers are the least desirable type of markers and are only used as a last resort.
The chemical content of the ink is monitored for acceptability as required by the ANSI standards.
e.
Other issuing controls 1.
Non-safety class material cannot be issued for safety class work.
Personnel verify that the issued safety class material is properly identified and marked with a red "P"
- tag, 2.
Issued material can only be used for the application identified.
Other usage required the return of the material for re-issue.
3.
Administrative controls are in place to insure main-tenance of the above during other than normal working
- hours, 4.
Unused safety related material returned to the Stores Ceoartment is required to be receipt inspected by the E igineering Support Department prior to being returned to stock.

Unused non-safety related material returned to the Stores Department is visually inspected for ser-viceability prior to returning it to stock.

Storage:
a.
Fasteners are stored in specified locations that provide the required protection for the item and accessibility for withdrawal when required. Safety class fasteners are stored in a manner suitable to the quality level identified in the purchasing document and in accordance with ANSI N45.2.2-1972.
Both safety class and non-safety class fasteners may be stored in the same general location but are subject to the controls described in C above.
1.
Storage areas are inspected on a quarterly basis for the following as applicable:
a.
Protective covers in place, b.
Temporary preservatives intact.
c.
Desiccants monitored.
d.
Identifiction and marking intact.
e.
Physical damage.
f.
Cleanliness.
g.
Access controls in force.
h.
Protection from rodents, fires, weather, or other factors that could damage stored items.
i.
Requirements for special storage addressed.
J.
Any unsatisfactory items discovered during the course of the inspection are documented and the corrective action addressed.
5.
Issuing:
a.
Fasteners are issued utilizing a material issue slip.
The following is recorded prior to issuance:
1.
Date and end use of the material.
2.
Authorization for use.
3.
Material description, part number, serial / heat / lot / batch numbers, safety class, purchase order.
4.
Quantity issued.
