Dear Mr. Counsil:

This refers to the inspection conducted by Mr. P. Stanish and other

NRC consultants during the period October 5 through November 1, i

1988, of activitics authorized by NRC Construction Permits CPPR-126

and CPPR-127 for the Comanche Peak Steam Electric Station, Units 1

and 2, and to the discussion of our findings with Mr. H. D. Bruner  !

and other members of your staff at the conclusion of the i

inspection.

The enclosed copy of our inspection report identifies areas

examined during the inspection. Within these areas, the inspection

consisted of selectivo examination of procedures and representative

records, interviews with personnel, and observations by the

inspectors.

During this inspection, it was found that certain of your

activities were in violation of NRC requirements, as specified in

the enclosed Notice of Violation. Two of the violations involved

(1) failure to properly evaluate a vendor design for suitability of

an application to moot project design assumptions and (2) failure

to ensure that the required technical dispositions for inaccessible <

inspection attributes would be performed as required by project i

procedures for soveral Field Verification Methods. In addition,

the NRC identified as an unrosolved item deficient design  ;

'

calculation reviews which failed to identify several instances of

incorrect design inputs, failed to follow established design  !

procedures, and omitted critical analyses. These findings are

considered to be a significant weakness in the piping and pipe

support area. A written response to the violations is required.

A management mooting to discuss the unresolved item will be  !

scheduled in the near future. i
