Thank you for your letter of October 6,1998, transmitting a KAY-RAY /Sensall incident report
- related to licensees' Model 7062BP, Serial No.18137V, Source Housing, and your letter dated October 09,1998 requesting additional information from KAY-RAY /Sensall. Per your request we have reviewed the KAY-RAY incident report and your letter dated October 09,1998. In l
. addition, we have searched Nuclear Materials Event Datebase, (NMED), for similar events. Our
. findings and recommendations are presented to you for your review and consideration.

Search and review of NMED identified as many as nine reported events related to Kay-.
Ray Model 7062BP and five reported events for Model 70628. To better understand this
~l data we have attributed three broad classifications to the reported events. The primary three causes of these accidents are vibrations, defective mechanical parts and loss of administration control. Loss of administration control events classification, is not design related and as such, was not given further consideration. The remaining events that attracted our attention were, two cases where the front face plate of the housing fell off, handles were missing in another two cases and sources became loose and fell out of l-the corroded source holder. A summary of a search is attached for your information and -
use.

The failure of a source holder source retention capability, missing shutter handles, and j'
corrosion attacks of source musing are significant indicators warranting reevaluation of the design and limiting conamons of applications of Models 7062BP and 70628. Also, housing may not be appropriate for use under general license. ( see10 CFR 32.51 (a)
_ (2)(ii) or any similar State regulations).

The KAY-RAY October 01,1998, event report indicates that the model 7062BP was manufactured in 1986. As stated in the KAY-RAY report dated October 01,1998, at that L
time manufacturer was loading the source capsule into an aluminum source holder. The l
licensee contends that brine vapors and/or the dust found in the area caused the aluminum source holder to be damaged. This assumption by the user does not appear
. to be adequately supported, if this premise is accepted, it would appear prudent for the user to provide detailed information and justification of continued operations and the

deployment of sources of 1986 aluminum design holders for use in harsh environments.

The State may wish to review Kay-Ray's plan related to modifications to mounting brackets to ensure uniform l
compliance with the intent of limitation 'C' of the registration certificate.
5.
Since the Models in question have suffered corrosion attacks and mechanical failures we recommend the licensee carry out nondestructive testing of failed materials or material indicating failure in progress to establish the nature of failures and the root causes and the contributing factors.
