SUBJECT: NOTICE OF VIOLATION
(NRC INSPECTION REPORT NOS. 50-259/88-21 50-260/88-21, 50-296/88-21)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
'
D.
R. Carpenter on July 1-31, 1988.
The inspection included a review of
-
activities authorized for your Browns Ferry facility. At the conclusion of the
inspection, the findings were discussed with those members of your staff
identified in the enclosed inspection report.
Areas examined during the inspection included operational safety, surveillance
observations, maintenance observations,
Plant Operations Review Committee,
reportable occurrences, restart test program, corrective action program,
condenser retubing, and General Electric contractor recommendations. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The inspection findings indicate certain activities appeared to violate NRC
requirements.
The violations, references to pertinent requirements, and
elements to be included in your response are described in the enclosed Notice
of Violation.
Note that Section 3 of the enclosed report identifies an unresolved item
pertaining to the adequacy of the administrative control over access to high
radiation areas (11000 mrem /hr).
We are concerned that the procedures
controlling the keys for access to high radiation areas are contradictory and
confusing.
In response to this letter please describe either why you believe
administrative control of these keys is adequate to meet the requirements of
Technical Specification 6.8.3.2, or what actions you plan to take to resolve
this concern.
