344/88-029, -030 Both trains of safety injection and one emergency diesel generator inoperable September 17, 1988 Trojan Summnary Both trains of the safety-injection (SI) system and one train of the emergency power system (EPS) were inoperable for 16 min.

one train of SI and one train of EPS were inoperable for approximately 48 h, and both trains of SI were inoperable for a period of 29.5 h.

One train of SI was inoperable because of clam debris in the lube oil cooler of the injection pump.

The conditional core damage probability associated with the event is 8.6 x 10-5.

The relative significance of this event com-pared with other potential events at Trojan is shown below.

Event Description Clam debris was found in the lube oil cooler of the SI train "A"l pump.

During the period when clam debris could have been in the lube oil cooler of train "A"l, the train "B" SI pump was inoperable on July 28-29 (28 h), August 26 (1.5 h), and August 29 (16 min).

The above informa-tion was provided by LER number 344/88-029.

In a related event (LER 344/88-030), two isolation valves in train "A" of the SI system were found in the wrong position.

The mispositioned isolation valves caused train "A" of the SI system to be out of service from June 3, 1988, until Sept. 17, 1988.

In addition, train "B" was out of service on July 13, 1988, Aug. 29 and 30, 1988, and Sept. 15 and 16, 1988.

The "B" train of emergency diesel generator was out of service on Aug. 29 and 30, 1988.

113-2 32 ASP Modeling Assumptions and Approach The event has been modeled as the sum of three periods.

The first period represents the 16 min on August 29 that both SI trains and one EDG train were inoperable.

The next period represents the approximately 48 h that one SI train and the opposite EDG train were inoperable on August 29 and 30.

The last period represents the period when both SI trains were unavailable for 29.5 h on July 28 and 29 and August 26.

In all cases, one train of SI was assumed failed because of the presence of clam debris in the lube oil coolers.

Analysis Results The conditional probability of severe core damage estimated for this event is 8.6 x 10-5.

The three segments that made up this event were (1) 2.8 x 10-7 for both SI trains and one EDG train inoperable, (2) 5.4 x 10-5 for one SI train and one EDG train inoperable, and (3) 3.2 x 1i-5 for both trains of SI inoperable.

The dominant sequence for all three cases involves a postulated small-break LOCA with unavailability of high-pressure injection and high-pressure recirculation.

This sequence is highlighted on the following event tree.
