Gentlemen:

This refers to the inspection conducted by Messrs. E. J. Ford and W. B. Jones

of this office during the period August 29 through September 15, 1988, of

activities authorized by NRC Operating License NPF-47 for River Bend Station,

and to the discussion of our findings with you and other members of your staff

at the conclusion of the inspection.

The area examined during the inspection was a followup on your staff's finding

that both divisions of the fuel building ventilation charcoal filtration system

was placed in an inoperable condition. This system is required to be operable

by the River Bend Station Technical Specifications. Within this area, the

inspection consisted of selective examination of procedures and representative

records, interviews with personnel, and observations by the NRC inspectors.

The inspection findings are documented in the enclosed inspection report.

As a result of this inspection, it was found that certain of your activities

appear?d to be in violation of NRC requirements. As discussed in the telephcne

conversation on September 26, 1988, between Messrs. G. L. Constable and L. A.

England of your staff, an enforcement conference will be scheduled in our

Region IV office, to discuss your understanding of these apparent violations,

the reasons for occurrences, and your corrective actions. We will also discuss

the enforcement policy, if appropriate.

The NRC staff is particularly concerned with ti:is event because it appears that

the corrective actions from a similar event 2 yurs earlier were not completed.

Had these earlier corrective actions been completed, this event could have been

avoided. In addition, your licensed operators shuuld have identified thet the

heater switches were misalligned during control board walk downs.
