the steps you have taken to correct the item of noncompliance regarding the

issuance of welding /c'utting permits which we brought to your attention in

Inepection Report No. 50-344/87-34.

During our review of your letter, we note that you acknowledged the validity-

of violation no. 87-34-02 concerning untrained supervisors issuing welding

and cutting permits.

We will examined your corrective actions for the

violation during a subsequent inspection.

Supplemental Information Concerning

Notice of Violation Item 87-34-02

This response provides supplemental information concerning Portland

General Electric's (PCE's) previous response to the subject item, as

,

committed to Mr. Chuck Ramsey of Region V during the NRC Fire

'

-

Protection Inspection exit meeting of April 21, 1988.

First, PCE confirms the validity of our original response to this issue

provided in the Response to a Notice of Violation letter, dated

December 28, 1987. At that time all supervisors issuing welding and

cutting permits were properly trained.

However, as noted by Mr. Ramsey during his April 1988 inspection,

untrained supervisors did issue welding and cutting permits during

April 1988.

The supervisors involved were coni.ract personnel recently

hired to augment the plant staff during the 1988 refueling outage.

Although they all have experience in welding and cutting activities,

they had not been provided plant-specific training in this area.

It

should be noted that all regular POE personnel of the Plant Maintenance

and Modification work groups who issued permits in 1988 were qualified

at the time of the inspection.

The causes of the event are twofold:

(1) Inadequate administrative

controls for ensuring proper qualification and training of applicable

craft personnel and (2) Inadequate procedural controls for issuing

welding and cutting permits.

Imediate corrective action taken included suspending the practice of

issuing permits on April 21, 1988.

Training was conducted and

documented for the previously untrained personnel. Temporary

administrative controls were established to require shift supervisors

to verify the training qualification of personnel issuing permits prior

to authorizing comencement of work. Subsequently, issuing permits

consueneed .

Long-term corrective action planned to avoid a repeated occurrence is

to proceduralize the practices of providing training and verification

thereof by the shift supervisor. Procedures will be revised by

June 30, 1988.
