Dear Sir:
This letter addicsses the status of our review and corrective efforts relative to the subject violations, both of which concerned the issue of locked / locking valv es.
Since our revised response of March 31, 1987 to Violation 414/86-18-01, we have implemented a program whereby those valves with straight bar handwheels are secured by the installation of factory-bought locking devices. During the last outages, we outfitted applicable valves inside and outside containment on Unit 2 and outside containment on Unit 1.
The work requests for the remaining inside containment valves on Unit 1 were put on a forced outage / trip list and j
will be completed as circumstances permit.
Our Design Engineering staff has completed a review of all locked valve / valve locking requirements, including the reasons for locking, the results of which have been incorporated into the Catawba Valve Database.
Since our response to Violation 413, 414/87-30-03 cated November 13,1987, we hava discussed the precipitating incident with shift personnel and re-emphasized locking procedures in our Nuclear Operator training. We have also taken measures to strengthen our administrative key controls for locked throttled valves.
In addition, we have issued an internal memo to station section heads to remind personnel who manipulate these valves to review the appropriate station documents for locked valve requirements.

- Document Control Desk June 1, 1988 Page 2 Initially, we anticipated that only a few procedures would require revision, but we increased the scope of-our review to re-evaluate all possible procedures, including those that deal with locked throttled valves. We are proceeding with these revisions, the first of which will be ready to implement within the next two months. We need additional time, however, due to the more extensive revisions required. We anticipate that the last of these revised procedures will be reats to be implemented by July 31, 1989.
