OS'ER FINDINGS AND COMMENTS Bolts & Nuts (B&N)
B&N is a material distributor of fastener products.
The company does not perform any manufacturing operations, and the Chattanooga office is the sole office for the company. Although B&N does not have a documented QA program, the president stated that B&N has done business since 1979, and their only nuclear custoe.er has been Tennessee Valley Authority (TVA).
BAN started supplying fasteners to TVA about six years ago. Their business (including fossil and nuclear) with TVA has been approximately 10. percent of total salos.
The inspector reviewed a TVA t.udit report dated February 28, 1985.
9&N's activities as an ' agent / supplier" were evaluated by TVA on January 29, 1985 for adequacy and implementation and found to be acceptable, The president indicated that this was the only audit performed by TVA.
When TVA purchases quality level fasteners which require certification a Paquest for Quotation (R/Q) is sent to B&N.
In the procurement documents-tion. TVA also specified vendors on their Approved Supplier List from whom B&N could purchase'the fasteners.
B&N then bids the order through one of the approved vendors. Subsequently. TVA issues a purchase order (PO) to B&N. The P0 toei.tifies the vendor along with a requirement that the fasteners along with a copy of a certified material test report (CMTR) or certificate of compliance (CofC) be sent directly to the nuclear site.
Copies of the certification documents along with a shipping invoice are sent to B&N.

PAGE 3 of 3 The inspector reviewed approximately 510 documentation packages for TVA orders from 1985 to the present.
Each package consisted of a B&N invoice to TVA, shippers receipt, vendor invoice to B&N, TVA RFQ and P0 along with specificatians, and manufacturer's CMTR and CofC. Of the 510 orders, approximately 100 orders were designated Quality Level II or III and were sent directly to the Browns Ferry, Bellefonte, Watts Bar, or Sequoyah nuclear sites.
Based on the QA requirements identified on each P0, it appears that Quality Level 11 is for safety-related fasteners (i.e., ANSI N45.2 and/or Appendix B to 10 CFR Part 50 were referenced) and Quality Level III (no QA requirements referenced) is for commercial fasteners.
For the Quality Level !! orders, it was noted that TVA did not pass on the requirements of 10 CFR Part 21 to B&N. The manufacturers of the Quality Level II fasteners were Texas Bolt and Cardinal Industrial Pro-ducts. Approximately 15 suppliers / manufacturers were the vendors for the Level III fasteners.
