FERMI INSPECTION REPORT 50-341/98013(DRP)

Dear Mr. Gipson:

On September.21,1998 the NRC completed an inspection at your Fermi 2 reactor facility.' The

enclosed report presents the results of that inspection.

During the 7-week period covered by this inspection, the plant operated at reduced power in

response to the closure of the No. 4. main turbine control valve. The No. 4 turbine control valve

was closed subsequent to a determination that the valve was contributing to secondary power

oscillations. - An evaluation by General Electric determined that the plant could operate safely at

reduced power with the No. 4 turbine control valve closed. During the plant shutdown for

Refueling Outage 6, another secondary power oscillation occurred. The operators in the control

room manually scrammed the reactor, and an investigation was conducted to determine the

'

root cause. This investigation determined that the power oscillations were caused by loose

valve actucion linkage for the No. 3 turbine control valve. Evaluation and analysis of the

degraded conditions of both valves was performed during the refueling outage.

In addition to the secondary power oscillations, the report discusses a number of work control

related deficiencies that contributed to human performance errors during the conduct of work

activities. Your continued attention to this aren is warranted.-

in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and the

enclosure will be placed in the NRC Public Document Room.
