SUBJECT: NOTICE OF VIOLATION
(NRC INSPECTION REPORT NOS. 50-348/88-05 AND 50-364/88-05)
This refers to the Nuclear Regulatory Comission (NRC) Operational Performance
Assessment Inspection conducted by S. D. Stadler and team on February 22 -
March 11, 1988.
The inspection evaluated the effectiveness of the Operations
group and other plant groups in supporting safe plant operations.
At the
conclusion of the inspection, the findings were discussed with those members
of your staff identified in the enclosed inspection report.
Areas examined during this inspection are identified in this report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews of personnel, and observation of
activities in progress.
The inspection findings indicate that certain activities violated NRC require-
ments.
A violation, references to pertinent requirements, and elements to be
included in your response are described in the enclosed Notice of Violation.
A violation described in the enclosed report, concerning installation of
nonconforming coupling bolts in the 1A and IB charging pumps is similar to Item
I.B of the Notice of Violation and Proposed Imposition of Civil Penalties for
NRC Inspection Report Nos. 50-348, 364/87-11 and 50-348, 364/87-14.
Your
response to Item I.B is being evaluated by the NRC.
Accordingly, a Notice of
Violation is not being issued for this additional example, and no response for
this item is required.
In addition, a violation, described in the enclosed
report, concerning hydrogen entrapment in RHR to charging pump crossover
piping, is under consideration for escalated enforcement action.
This item
will be discussed during a future Enforcement Conference.
However, please be
advised that the number and characteristics of violations described in the
enclosed report may change as a result of further NRC review.
You will be
advised by separate correspondence of the results of our deliberations on these
matters.

We are concerned that you had identified safety-related pump room coolers as
support equipment rather-than attendant equipment.
Unless you can provide an
analysis that indicates these coolers would not be required for pump opera-
bility during accident conditions, we consider the safety-related pump room
coolers to be attendant equipment.
It is our understanding that you will
treat safety-related room coolers as attendent equipment under the Technical
Specification definition of OPERABLE - OPERABILITY either permanently or until
an analysis is submitted and reviewed by the NRC.
Your attention is invited to an unresolved item identified in the inspection
report concerning General Manager approval requirements to minor departures.
This matter will be pursued during future inspections.
In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2,
Title 10, Code of Federal Regulations, a copy of this letter and its enclosures
will be placed in the NRC Public Document Room.
The responses directed by this letter and its enclosures are not subject to the
clearance procedures of the Office of Management and Budget as required by the
Paperwork Reduction Act of 1980, PL 96-511.
