NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/88-09 AND 50-328/88-09)

This refers to the Nuclear Regulatory Comission (NRC) inspection conducted by

J. J. Lenahan on January 25 - March 17, 1988.

The inspection included a review

of activities authorized for your Sequoyah facility.

At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of selective examinations of procedures

I

and representative records, interviews with personnel, and observation of

'

activities in progress.

The inspection findings indicate that certain activities appeared to violate

NRC requirements.

The violations, references to pertinent requirements, and

elements to be included in your response are described in the enclosed Notice

of Violation.

Violation A involves installation of the improper type of

structural steel bolts during the seal table modification.

Violation B

involves storage of quality assurance records in a facility which does not

comply with the requirements of your QA program.

With respect to Violation B, we have concerns that members of your staff

consider it acceptable to improperly store documents important to safety for

periods in excess of six months, solely on the basis that a final approval

signature for those documents remain outstanding, and therefore, the documents

are not yet classified as "quality assurance records".

Th mere fact that the

final authorized signature had not been affixed to the wori plan cover sheet to

indicate completion of the total work plan, does not negate tne importance of

preservation of the individual documents (quality assurance records) within the

work plan packages.

Additionally, we have concerns that internal disagreements within your

organization have prevented timely resolution of this problem through your own

resources.
