APPENDIX A NOTICE OF VIOLATION Texas Utilities Docket No. 50-446 Comanche Peak Unit No. 2 License No. CPPR-127 An NRC inspection conducted during the period July 25 through August 5, 1988, identified one violation of NRC requirements. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the particular violation is set forth below.

10 CFR 50, Appendix 8, Criterion XV, Nonconforming Materials, Parts, or Components, requires that a system be established to control materials, parts, or componenets which do not conform to requirements in order to prevent their inadvertent use or installation.

Site Quality Manual Section 16.0,Nonconforring items paragraph 16.2, directs all site employees to report items of nonconformance in accordance with approved project procedures.

Contrary to the above, an NRC review of pipe weld radiographs during July 25 through August 5, 1988, disclosed that ASME Class 2 Spool No. 51-2-YD-03-6, which had been accepted and installed by the licensee, contained severe undercut (approximate 40 linear feet) that had oeen identified in licensee radiographic report 5546 but never was evaluated or dispositioned as a nonconforming item.

This is a Severity Level IV violation (Supplement II) and was identified for Unit 2, Occket No. 50-446.

Pursuant to the provisions of 10 CFR 2.201, Texas Utilities is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
