Gentlemen:

Subject: Inspection Report No. 50-271/86-05

This refers to the special inspection conducted by Messrs. W. J. Raymond and

G. W. Meyer of this office on February 9-27, 1985, at the Vermont Yankee Nuclear

Power Station, Vernon, Vermont. . The inspection was performed as a result of the

Standby Liquid Control (SLC) system test failure on February 8, 1986. The inspec-

tion findings were' discussed with Mr. J. Pelletier of your staff at the conclusion

of the inspection.

Our review of the circumstances associated with the SLC system test failure indi-

cates the system was inoperable from July 11, 1984 until testing on February 8,

1986 due to an unintended change in the squib primer wiring by the vendor.- We are

concerned that your procurement program and post-installation testing program did

not detect a product change that affected the operability of a safety system. We

are also concerned with an unapproved change which you made to the squib firing

circuit..

The apparent violation identified in our review of this event is discussed in the

enclosed report. The need for and nature of appropriate enforcement action rela-

> tive to the apparent violation will be the subject of separate correspondence at

, a later time. These matters will be discussed in an enforcement conference with

} you and members of your staff scheduled for March 19, 1986. You should come to

the meeting prepared to discuss ~your understanding of these events and your planned

and implemented corrective actions,
