Inspection 50-423/85-74 (11/19/85-1/6/86)
This refers to the routine inspection by Messrs. T. Rebelowski, F. Casella, and
J. Shediosky of this office from November 19, 1985 to January 6, 1986 at the Mill-
stone Point Nuclear Power Station, Unit 3, and to the discussions of findings with
Mr. Crockett and other members of your staff during and at the conclusion of the
inspection.
It appears that certain of your activities ' violated NRC requirements as set forth
in the Notices of Violation enclosed as Appendices A and 8.
The Violations of
Appendix B contain Safeguards Information and will not be discussed in this letter
except to state that we believe both are isolated cases.
The Violations in Appendix A are indicative of problems which, if uncorrected,
could become safety concerns.
The first identifies lapses in cleanlivess control
when safety related systems are open.
In your response, please include details
of long term corrective actions to enhance personnel knowledge and understanding
of cleanliness requirements. The second Violation in Appendix A concerns a failure
to follow procedures and to take thorough corrective ar.tions when a Plant Equipment
Operator found a Diesel Fuel Oil System valve mispositioned.
This did not result
in esragement directed follow-up to evaluate the reason for the mispositioning and
to assess the potential for other valves being out of position.

this in your response.
