APPENDIX A NOTICE OF VIOLATION Omaha Public Power District Docket: 50-285 Fort Calhoun Station Operating License: DPR-40 Ouring an NRC inspection conducted during the periods July 25-29 and August 15-19, 1588, violations of NRC requirements were identified. The violations involved the (1) failure to mark component cooling water pump bearing housing, AC-3, for locations of vibration measurements and (2) failure to control and calibrate stopwatches being utilized in stroke-timing valves tested under the IST program. In accordance with the "Ge'Jral Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2. Appendix C (1988), the violations are listed below:

A. 10CFR50.55a(g)(4)requiresthattheappropriateprovisionsofASME Code,Section XI be applieu during conduct of inservice tests of the l operability of pumps and valves.Section XI Subarticle IWP-5420 requires that vibration reference points be clearly identified to permit subsequent duplication of measurements in both plane and location.

In violation of the above, there were no discernible markings on component cooling water pump bearing housing, AC-3, that would indicate the requirements of IWP-5420 had been complied with.

B. Criterion XII of Appendix B to 10 CFR Part 50 requires that tools, gauges, instruments, and other measuring and testing devices used in activities affecting quality are properly controlled, calibrated, and adjusted at

specified periods to maintain accuracy within necessary limits. The Fort j Calhoun Station, Standing Order M-28. "Calibration of Test Equipment and Plant Process Equipment Used To Support the Inservice Inspection of Nuclear Plant Components Program," identifies stopwatches to be included in the control of the calibration program, Contrary to the above, the NRC inspector observed that stopwatches utilized by control room operations personnel for the purpose of full-stroke timing of valves prescribed by the IST surveillance program, lacked evidence of control numbers or calibration due dates as prescribed by Standing Order M-28.

Since corrective action was initiated, completed, and verified prior to the conclusion of the NRC inspection, no written response is necessary.
