TWO HISSING IODINE 131 CAPSULES Region I was notified on September 12, 1988 by E. R. Squibb and Sons regarding two missing capsules, each containing 0.5 millicuries of Ivdine-131 that were shipped to Roswell Park Memorial Hospital in Buffalo, New York on September 7, 1983.
Region I and the State vf New York have investigated the report with the two licensees and the NYS Health Agency sent an inspector to the hospital on September 12.
It is suspected that the sources tray have been disposed inadvertently with the shipping package.
